F/41 yrs, multiple cervical lymph nodes, lymph node biopsy.

Partially effaced archtitecture with few atypical cells which are cd 30 positive but cd15 negative.

This is a very difficult case, which needs additional studies.  
 
Please have a closer look if the atypical lager cells are CD20+!  
 
Please add MUM1 and PAX5 as well as an EBV stain (EBER or LMP1) to estimate if the respective cells are positive!  
 
Please have a closer look at CD5 if there is a rimming around the atypical cells!  
 
What about the clinical presentation and the CT scans? Is there a bulk, does the patient have any paraneo0plastic symptoms? Is there a history of recent infection, incl. mononucleosis?  
 
How large are the lymph nodes? Is there a capsular desruction of the lymph nodes? Are there necroses?

I agree this is not an easy case. In the DD I would consider lymphocyte rich cHL.
