I consider a marginal zone lymphoma, mantelcell lymphoma or follicular lymphoma with a preference for the first two options. I recommend IHC using CD20, CD21, CD3, CD5, Cylin-D1, Bcl-6 and CD10.

I have uploaded IHC images.
I think IHC is in favor of follicular lymphoma.
Central area pf the lymph node is givin faint rection in cbcl2 and few other antibodies. (?due to fixation effect)

I agree with you that the IHC favors a follicular lymphoma grade 1, rather than a mantelcell lymphoma (CD5 negative, CyclinD1?) or a marginal zone B-cell lymphoma. This outcome surprises me a little bit because of the lack of blasts in the high power images and the high turnover in the Ki-67 immunostain.

I agree with a diagnosis of follicular lymphoma, you should carefully count the blasts in the Giemsa stained slide, since Ki-67 is indeed increased.

CyclinD1 is negative, CD20 is positive and Blast is between 20% to 50% (Mixed category).
Thanks a lot Dr Vrugt and Dr Tzankov
