Clinic:  Bladder outlet obstruction.
 
Gross:  Spherical   specimen,  diameter  4  cm,  30g,  firm   to hard.
 
Cut:    Central nodularity  surrounded  by  a  shell of   compressed    parenchyma  (gross,   4472). 
 
Problem: Within  the    compressed    parenchyma  a   focus (4466) of    atypical  glands  with   prominent  nucleoli and  features    suggestive of   dissecting   the  surrounding  fibers.  Thanks !

The configuration of the atypical glands justifies in my view the diagnosis of a small prostatic adenocarcinoma with a Gleason pattern 3.

This is a Gleason 3 Pseudohyperplastic adenocarcinoma imitating an exuberant hyperplastic nodule.it consists of large atypical glands with infoldings.
