Problem:   I hesitate  to make   a   diagnosis  of   a   squamous  cell   carcinoma  in   this  young   women  and   a history  of    8   years,  but  what   would  be    the   alternative ? In  addition      there  is  a   swelling of  the   axillary  lymph  nodes of  the  same  side:  "smear    shows     cytologic    features  of    malignant  round  cell  tumor  with   prominent  nucleoli  and  moderate   cytoplasm;  it looks    more    secondary    than  Hodgkin's  lymphoma".

kunze 2011-12-22 15:07	The clear cell component in the last image and the anastomosing strands of spindle-shaped atypical epithelial cells with an intimate connection to the epidermis and central nests of well differentiated squamous epithelium lead me to the diff. diagnosis of a malignant sweat gland tumor, especially a porocarcinoma, which corresponds better with the acral localisation and a long clinical history.
