42 years old Japanese patient suffered from marked pancytopenia:  
WBCs : 2100/ul  
RBCs : 3.1 Millions/ul  
PLATELETS: 29000/ul  
 
CT HEAD,NECK,CHEST,ABDOMEN & PELVIS ARE UNREMARKABLE.  
 
* IMMUNOSTAINING RESULT:  
- NEGATIVE CD10, BCL-6, PAN-CK,CD138, EMA, CD3, CD79a, LOW Ki67 [4%].  
- POSITIVE: CD45, VIMENTIN, BCL-2

poorly dehydared BMB with diffuse infiltration by atypical discohaesive eosinophilic cells with hematopoietic displacement. I would suggest going on with the characterization of the cells. a nice Gimesa staining as well as some additional palsma cell markers (CD38, MUM1, Kappa, Lambda, even CD56 and/or cyclin D1) and myeloid and histiocytic markers (CD11c, CD14, CD68, CD163, MPOX, CD15) would be helpful. are there any ostelyses in the CT? M-gradient? Which pan-CK did you use?
