Patient:27 years old lady with abdominal swelling and splenomegaly. Hb-9, HCT 29.9 Platelet 43.  
 
Male or Female: Femaled  
 
Age: 27 yrs  
Clinical information:  
 
Question: No clear evience for aplastic anemia/ Slight hypercellular normal marrown

I see a hypercellular bone marrow with relative erythropoietic hyperplasia and synchronization and slight megakaryopoietic left shift. The changes might be explainable by hypersplenism per se, but might be die to (autoimmune, infectious or corpuscular) hemolysis or be due to both. To solve the case we need to know:  
1. Duration of the disease?  
2. Size of the spleen?  
3. History of medication?  
4. History of infections?  
5. Exact results of the differential blood counts.  
6. Comb's test results. LDH? ESR? Haptoglobin?  
7. Family history of similar diseases.  
 
Aplastic anemia, bone marrow infiltration by an expansive non-hematopoietic process and overt blastic disease can already be excluded.

The splenomegaly is for almost three years. There is para-aortic lymphadenopathy present, core biopsy of which was non conclusive. The spleen by MRI show masses, but ultrasound show homogenous spleen with irregular borders where no mass is visualized.
