Male 40yrs with Leukocytosis,WBC of 119, morphological suggestive of CML,Hb-9g/dl, Platelets-104, aspirate was a dry tap, Trephine pics attached, any possibility of blast transformation?

The granulocytic elements reveal a shift to the left, but up to the highest magnification provided, nuclear details are not very clear to count blasts. IHC for CD34 & CD117 will help to confirm

There is a marked blood leukocytosis (119): the blood film morphology, including differential count and percent of blast cells, is important (if the precent blast cells Is 20 or more, this defines blast phase).  
 
There is no morphological evidence of blast phase of the chronic myeloid leukaemia on the sections of the bone marrow.

Thank you Samia and Erber for the comments,the differentaial count showsBlasts-2%,promyelocytes-8%,Myelocytes-15%,bands-4%,neutrophilis-25%,eosinophils-4%,basophils-8%,lymphocytes-10%.Unable to do the CD34 and CD117 currently.He is currently on CML treatment and due for BCR ABL1 test, but we are following up closely with serial CBC and PBF
