This case is uploaded for access at the patient's distant hospital for diagnostic use ant treatment decisions.  
 
51 yrs old male. Weakness, sweats, 10kg loss in several months. 2014 12 admitted to hospital for evaluaton.  
 
Threpine biopsy: Threpine biopsy with cellularity up to 100%. Up to 80% BM space is occupied by difusse and partially vaguelly nodular lymphoid infiltrate with small-medium sized cells with ovoid and slightly cleaved nuclei with scattered small nucleoli and scarse cytoplasm. Small lymphocytes are located outside nodules in "matle" zone.  
Residual haematopoiesis: M/E ratio 3/1, peritrabecular NASDE+ granulopoiesis islands. and more central erythron islands Medium sized megakariocytes up to 7/ HPF. Reticulin fibrosis absent.  
 
FLOW: ~86% venous B lymhocytes (CD19+): CD45+, CD5+ dim, CD19+, CD22+ dim, CD20+ strong, HLA-DR+, CD79b+ medium, CD38+, CD23-, sIgM-, CD43-, CD10-, Ig kappa:lambda=1:66, lambda > kappa: compatible with B MCL.  
 
Diagnosis: B mantle cell lymphoma (difusse- nodular pattern; conventional variant), 80% bone marrow space.  
Immunophenotype (typical): CD20+; CD5+; CyclinD1+; CMYC(-); CD23/Bcl6/CD10(-); Ki67 ~ 7%.
