I see a hyperplastic tonsil with follicles and progressively transformed germinal centers without obviosu evidence of a lymphomatous spread.how large was the tonsil? does the patient suffer from a malignancy?

dr. Tzanakov, this is a piece of left tonsil tissue. Surgeon told me that whole left tonsil recently growth very fast and he took just piece of it. The right tonsil is calm and regular in size.We didn't receive any further information about this patient cause this biopsy is revision from other city.  
We are planing to stain EMA, CD30, CD21, CD23, BOB1 i OCT2 'cause Ki67 is highly proliferative and be sure not to miss any type of malignancy.  
Are you agree and do you have any other suggestion?

5 cm is indeed very large. can you please add CD2 or CD7 and kappa and lambda, since there is a parafollicular population that is still not stained by CD3 nor by CD79a. why BOB1 and OCT2? these markers have a very limited utulity in the DD of Hodgkin...

lambda is predominant, but still not clonal. I do not see any follicular colonization or sheets of interfollicular B-cells to suspect marginal zone lymphoma. weren't there FDC in the CD23 staining? does the patient have an M-gradient?

Very sorry but we don't have that type of clinical information cause this biopsy is from other part of our country. we've just received paraffin blocks and slides for immunostaining and supervision of previous diagnosis NHL.

we will finish this like "highly suggests progressive transformation of germinal center" and ask for comparation with clinical data.
